Laser Surgery vs. Ksharasutra:
Choosing the Right Treatment for Your Piles

Haemorrhoids (piles) affect an estimated 75% of people at some point in their lives. When conservative management — dietary changes, sitz baths, Ayurvedic medications — fails to resolve them, two minimally invasive options dominate modern proctology: Laser Haemorrhoidoplasty (LHP) and the classical Ksharakarma / Ksharasutra technique. Both offer major advantages over open haemorrhoidectomy, but they suit different patients, different grades and different lifestyles.

Understanding Piles: Grades Matter Most

Before comparing treatments, grading is essential — the grade of haemorrhoids determines which approach is appropriate:

Grade
Description
Best approach
I
Bleeding only, no prolapse
Ayurvedic medicines + diet
II
Prolapse on straining, spontaneous return
Ksharakarma or laser
III
Prolapse requiring manual reduction
Laser or Ksharasutra
IV
Permanently prolapsed
Laser haemorrhoidoplasty

How Laser Haemorrhoidoplasty Works

In LHP, a radial laser fibre (980 nm or 1470 nm) is inserted into the haemorrhoidal tissue through a tiny puncture. The laser energy is delivered in precise pulses, causing coagulative necrosis of the submucosal haemorrhoidal plexus without cutting any tissue. The pile shrinks and is reabsorbed over 4–6 weeks. The procedure takes 15–20 minutes under local or spinal anaesthesia.

How Ksharakarma Works for Piles

Ksharakarma applies Apamarga Kshara (alkaline herbal paste) directly onto the haemorrhoidal mass through a proctoscope. The alkaline pH causes controlled chemical cauterisation — the pile gradually desiccates, sloughs off, and the underlying vessels are permanently obliterated. This is a true non-surgical approach with no incision, no anaesthesia requirement, and zero risk of post-operative bleeding.

Which Should You Choose?

The honest answer: it depends on your grade, constitution and lifestyle. At Yathartham Shalyaveda, Dr. Piyush follows a clear decision framework:

Our Non-Surgical First Principle

We never rush to surgery. Over 60% of patients presenting with Grade I–II piles at our clinic are successfully managed without any procedure through a 6–8 week Ayurvedic protocol of herbs, diet and posture correction.
