Irritable Bowel Syndrome — Diarrhea predominant (IBS-D) affects millions of people who cycle through loose stools, urgency, bloating and cramping. Modern gastroenterology offers symptomatic relief but rarely addresses the underlying disruption. Ayurveda, however, identifies IBS-D as primarily a Vata-Pitta imbalance — excessive movement (Vata) combined with inflammatory heat (Pitta) — and treats both simultaneously through a unique rectal therapy called Takra Basti.
What Is Takra Basti?
Basti is the Ayurvedic term for medicated enema therapy — one of the five primary Panchakarma detoxification procedures. Takra refers to medicated buttermilk. Unlike conventional enemas that only cleanse the colon, Takra Basti delivers a warm, medicated buttermilk formulation that:
- Coats the inflamed colonic mucosa with a protective, cooling film
- Normalises peristalsis by pacifying aggravated Apana Vata
- Removes accumulated Pitta from the large intestine
- Replenishes the intestinal microbiome through natural Lactobacillus cultures in fresh buttermilk

Takra Basti is administered as a 15–21 day course in most cases, with daily or alternate-day sessions depending on severity:
- Days 1–3 (Preparation): Oleation (Snehana) with medicated ghee orally to mobilise Ama from deep tissues
- Days 4–15 (Core Basti): Alternating Anuvasana Basti (oil-based) and Niruha Basti (decoction-based including Takra Basti)
- Days 16–21 (Consolidation): Oral herbs — Kutajarishta, Bilwadi Churna — to sustain the colonic environment
Clinical Results: What Patients Experience
In our clinical experience, patients undergoing the full Takra Basti protocol for IBS-D typically report:
Observed Patient Outcomes
Significant reduction in stool frequency within 5–7 days. Complete resolution of urgency in 70–80% of patients by the end of the 21-day course. Sustained improvement at 3-month follow-up in approximately 75% of cases — provided dietary guidelines are maintained.
The Prakriti-Based Diet That Sustains the Result
Basti alone is not enough. The diet prescribed alongside it is as important as the procedure:
- Include: moong dal, rice kanji, cooked vegetables, buttermilk (not cold), pomegranate, banana
- Avoid strictly: raw salads, cold foods and drinks, red chilli, fermented foods, coffee, alcohol, late night eating
- Lifestyle: fixed meal times, 10-minute left-lateral rest after lunch, oil pulling (Kavala) each morning
