Why Ksharasutra Has the Lowest Recurrence
Rate (<1%) for Fistula-in-Ano

Fistula-in-ano — an abnormal channel between the anal canal and the perianal skin — is one of the most frustrating conditions in proctology. Conventional surgery cuts the tract open (fistulotomy) or removes it (fistulectomy), yet recurrence rates of 20–40% are routinely reported in literature. Ksharasutra, the medicated thread described in Sushruta Samhita over 2,500 years ago, achieves something conventional surgery cannot: it heals the tract from inside out, layer by layer, while leaving the sphincter untouched.

What Is Ksharasutra?

Ksharasutra literally means "alkali thread." A surgical-grade linen thread is repeatedly coated with three medicinal agents in precise sequence over 21 days, creating a layered thread with pharmacological action built into its structure:

The completed thread has a pH of approximately 9–10, which is precisely calibrated to dissolve fibrous fistulous tissue without damaging normal mucosa or sphincter muscle.

The Procedure: Step by Step

How Does It Achieve <1% Recurrence?

The key to Ksharasutra's extraordinary outcomes lies in its simultaneous triple action:

The Three-Action Mechanism

Chedan (cutting) — the thread mechanically cuts through the fibrous tract. Bhedan (splitting) — alkaline cauterant opens up cavities and side-branches. Lekhana (scraping) — the thread's texture physically debrides the tract lining with every change, preventing re-epithelialisation of the channel.

Conventional surgery achieves only cutting. It removes the tract but leaves behind an inflammatory microenvironment that allows new tract formation — hence high recurrence. Ksharasutra's chemical action sterilises this environment while the mechanical action ensures no residual lining survives.

Ksharasutra vs Conventional Surgery: A Comparison

FactorKsharasutraConventional Fistulotomy
Recurrence rate<1%20–40%
Sphincter injury riskNegligibleModerate to high
Incontinence riskVirtually zero5–15% (temporary or permanent)
Hospital stayOutpatient / same day1–3 days

Pain levelMild, managed with oral medsModerate to severe

Who Is a Candidate?

Ksharasutra is indicated for all grades of fistula-in-ano, including complex, recurrent and high trans-sphincteric tracts where conventional surgery poses unacceptable incontinence risk. It is particularly valuable for patients who have already undergone failed conventional surgery.

⚕ Contraindications include active Crohn's disease with severe perianal inflammation, immunocompromised states (uncontrolled diabetes, active TB), and pregnancy. Dr. Piyush will assess your suitability at consultation.

What to Expect at Yathartham Shalyaveda

At our clinic, the Ksharasutra thread is prepared in-house using classical protocols under strict aseptic conditions. Thread changes are performed every 7–10 days as an outpatient procedure lasting under 15 minutes, with full aftercare guidance including sitz bath protocols, diet modifications and follow-up sigmoidoscopy.
