An anal fissure is a small tear or cut in the lining of the anus, typically caused by passing hard or large stools. It leads to severe, sharp pain and bright red bleeding during and after bowel movements.

Common clinical signs of a fissure include:

  • Sharp, cutting pain during defecation, which can persist for hours due to sphincter spasms.
  • A visible tear or crack in the skin around the anus.
  • Bright red blood on the stool or toilet paper.
  • A small lump or skin tag (sentinel tag) near the tear in chronic cases.

Causes and Complications

Fissures arise from local trauma to the anal mucosa. Primary causes are chronic constipation, straining, or persistent diarrhea. If left untreated, chronic fissures can lead to severe anal spasms, infection, and fibrous sentinel tags that require surgical excision.
